1) double-click "My Computer", go to C: and right-click a file, it will give you
an option "Select" (which is default). "Select" should only appear in File
Save/Open dialogs, not in Explorer.

2) Also there's "Open" and "Explore" (this is on an inf file I'm testing), which
shouldn't be there. "Explore" should only be on folders, and "Open" should only
be on folders and items that have an "Open" verb defined ( which this extension
does not). Finally, verbs that ARE in HKCR aren't showing up. (in particular I
created an "Install" verb for .inf files, and it isn't showing up in the context
menu)
