Forgot 3 things

1. Another of my changes which changes an ASSERT to a DPRINT because this error
doesn't seem to have any adverse effects.

2. And another change replaces IO_NO_INCREMENT with IO_DISK_INCREMENT because
we just completed disk I/O. IO_NO_INCREMENT is for errors and requests that can
be served quickly.
