I found a way to replicate this bug in VMware:
1. Open the BIOS by pressing F2
2. Confirm that Legacy Diskette A: is set to: 1.44/1.25 MB 3 1/2. If not press
- while it is selected until it is set correctly.
2. Press the right arrow key to move to the Advanced tab.
3. Use the down arrow to go down to I/O Device Configuration.
4. Press enter to enter the I/O Device Configuration page.
5. Use the down arrow to go down to Floppy Controller.
6. Press - to change the value to Disabled.
7. Press F10.
8. Press Enter.
When it reboots, Press F1 to continue booting.
I also got a debug log that ends with this, (ntoskrnl/io/pnpmgr/pnpmgr.c:2558)
IopInitializeDevice(Root\*PNP0303\0000) failed with status 0xc0000001, which is
(oddly enough) normal. Therefore, I think this hang is caused by an endless
loop somewhere in floppy.sys and that would be why it never shows a BSoD.
