polish translation patch

Mostly syncing with trunk. Also contains a fix for usetup language files -
en-US file was missing a "Setup" word at the beginning of ERROR_WRITE_BOOT
string. It can be easily noticed by comparing it to the similar
ERROR_INSTALL_BOOTCODE string.
