That's a fairly serious problem.  Even Microsoft's own NT4 DDK used a 16-bit
setup app, IIRC.

These things are all over the place in the form of things like
InstallShield.  Microsoft even implemented some special processing of
InstallShield packed setup scripts for Windows on IA-64 and x86-64 due to a
lack of 16-bit execution compatibility.
