fixes a few msvc compilation issues and breaks it gcc support

apply patch w/o first patch, then ntoskrnl.exe nearly compiles -> but gcc will
fail -> 
compilation fails in: 
ke\spinlock.c
ke\process.c
mm\npool.c
kd\kdmain.c
kdbg\kdb_cli.c
ke\i386\fpu.c
io\disk.c
io\bootlog.c
