OK. Here is a correct one:

Hardware: Tried with vmware player, virtual pc and qemu sometimes with kqemu.
Version: r20545 (svn)
What was I doing: It happened during numerous thingsP
a) downloading files with firefox
b) running files of a cd
c) shutting down

Note that the download error does not occur with ibrowser.

BSOD: See screenshot

Version of apps: ibrowser that came with reactos, firefox 1.5, reactos version 
noted above.
