second stage install crash with backtrace

the thing is this only happens on real hardware.  it works fine in qemu.  also,
it started after 32546. 32497 was fine. i've got a build of 32446 i've been
using when i want to play.  it installs and boots just fine.

- Implement MiShutdownMemoryManager (stops MPW thread and writes all dirty
pages to disk).
- Move MiShutdownMemoryManager invocation to where it should really happen
(before filesystems shutdown).
- As a result, MPW doesn't have a chance to flush pages when filesystems are
already down. However, 1st assertion as mentioned in bug 2872 still happens.
2nd one is gone.
