Introduced with UNIATA in rev.29253, temporarily fixed in rev.40896, then fix
was reverted in rev.43921

It might affect 1st stage as well as 3rd and later. Usually happens while CDROM
is heavy with I/O operations (lenghty file copying) but on some systems it
might even be triggerd with accessing CDROM in explorer. The suspected
mitigation factors are cpu speed/age on virtualbox

Its unknown if it happens anywhere besides VirtualBox.

This problem mostly occurs with a pair of:

(drivers/filesystems/cdfs/common.c:107) STATUS_VERIFY_REQUIRED
(drivers/filesystems/cdfs/fsctl.c:577) CDFS: IRP_MN_VERIFY_VOLUME
(drivers/filesystems/cdfs/fsctl.c:454) CdfsVerifyVolume() called
(drivers/filesystems/cdfs/fsctl.c:475) Volume has been verified!
(drivers/filesystems/cdfs/common.c:112) IoVerifyVolume() returned (Status 0)

but other issues seem to be connected. One of them is smss.exe fatal exception,
plus few other crashes, that present file copying functions in backtrace.
