Official bootcd-37497-dbg.iso.

open cmd, do "rename notepad.exe NOTEPAD.exe", you get:
notepad.exe -> NOTEPAD.exe
MoveFile() failed. Error: 80
    0 Files renamed

Same happens for extensions.
